Qualtrics
UW Qualtrics is an online data collection tool licensed by the University of Washington for use in academic-related research and administrative projects. UW Qualtrics grants access to Qualtrics Core XM features, allows users to create and administer surveys and web forms, and provides quantitative analysis and reporting on the collected data.
How to Get Qualtrics
Step 1: Check for an Existing Qualtrics Division (Required)
Most departments already have a division relevant to your needs. Check Existing UW Qualtrics Divisions
Any UW Qualtrics division administrator can grant your account access by adding it to their Qualtrics division.
If you find a relevant division, contact the listed administrator to request access.
Do not skip this step. Submitting a request without checking whether a division exists may delay fulfillment of your access request.
Step 2: Request a New Division (If Needed)
Only complete this step if no existing division meets your needs.

When using the UW Qualtrics Brand, you must follow the University’s Privacy policies, which were developed by the UW Office of Information Security and UW Privacy Office.

Restrictions
The UW Qualtrics Brand is strictly limited to purposes that support the university’s mission, including but not limited to academic and research purposes. The use of Qualtrics for personal activities is prohibited.
When collecting survey data, consider legal, regulatory, and university policy requirements. If you are unsure whether your survey questions contain or request sensitive data, contact the UW Privacy Office for guidance.
Extensive university-wide surveys of population groups such as faculty, staff, researchers, and students require advance approval from the Office of the Provost. Any user who violates this rule will have their Qualtrics account terminated. Please review UW’s measures to protect information, as described in the UW Administrative Policy Statements 2.2, University Privacy Policy, and 2.6, Information Security Controls and Operational Practices.

Division Administrator Replacement
If you are a current Division Administrator who will be changing responsibilities or leaving the university, your division must maintain at least two administrators.
Contact the UW-IT Software Licensing team by emailing help@uw.edu with the subject “Qualtrics division changes.”
Please provide your replacement division administrator’s contact information and copy them on your message.
